Laboratory studies have shown inhibition of both type I and type II 5-alpha-reductase, while clinical studies have evaluated saw palmetto directly in people with androgenetic hair loss.
In a 24-month clinical study of 100 men with mild-to-moderate androgenetic alopecia, participants received either 320 mg per day of dry Serenoa repens extract or 1 mg per day of finasteride.
After two years:
- 38% of the saw-palmetto group showed increased hair growth.
- 52% had stabilization of their hair loss.
- 10% worsened.
- Finasteride produced improvement in 68% of participants and was clearly the stronger treatment overall.
That is an important distinction: saw palmetto did not perform as strongly as finasteride, but 90% of the saw-palmetto group either improved or remained stable over the two-year period.
More recent randomized research also shows that highly concentrated saw-palmetto extracts can produce measurable improvements at considerably lower doses. A 2026 placebo-controlled trial using 160 mg per day found significant improvements in terminal hair count and overall hair density in men and women after six months.

Pumpkin seed oil is another botanical with human clinical evidence relevant to androgen-related hair loss.
In a randomized, double-blind, placebo-controlled trial involving 76 men with androgenetic alopecia, participants took 400 mg of pumpkin seed oil daily for 24 weeks.
The men receiving pumpkin seed oil experienced an average 40% increase in hair count, compared with approximately 10% in the placebo group.

The saw-palmetto and pumpkin-seed-oil numbers should not be compared as though they are the same measurement. The saw-palmetto study reported the percentage of people who improved or stabilized, while the pumpkin-seed-oil trial measured the average percentage increase in hair count.
Both have legitimate clinical evidence. For a multi-ingredient capsule, however, a concentrated standardized saw-palmetto extract offers a significant practical advantage because it can provide androgen-focused support with a smaller formulation footprint.
SERUMS AND TOPICAL DHT SUPPORT
Many scalp serums contain caffeine, peptides, botanical extracts, or other cosmetic ingredients. Some may improve scalp condition or hair quality, but simply applying an ingredient to the scalp does not make it an effective DHT treatment.
Topical finasteride is different because it directly inhibits 5-alpha-reductase at the scalp.
In a Phase III randomized trial involving 458 men with androgenetic alopecia, topical finasteride produced a significantly greater increase in target-area hair count than placebo after 24 weeks. Its improvement was numerically similar to oral finasteride, while maximum blood concentrations of finasteride were more than 100 times lower and serum DHT suppression was lower with topical treatment.

Finasteride inhibits type II 5-alpha-reductase, reducing the conversion of testosterone into DHT. Oral finasteride 1 mg is FDA-approved for male pattern hair loss.

Dutasteride
Dutasteride blocks both type I and type II 5-alpha-reductase, producing broader DHT suppression than finasteride.
In a randomized study of 917 men with androgenetic alopecia, dutasteride 0.5 mg produced significantly greater improvement than finasteride 1 mg in several measures of hair count, hair width, and photographic hair growth after 24 weeks.

ADDRESS THE TRIGGER
“Stress” does not mean only emotional stress. Hair-cycle disruption can follow many different stresses on the body, including:
- prolonged psychological stress
- serious illness or high fever
- surgery or physical trauma
- childbirth and postpartum hormonal changes
- rapid weight loss
- rapid weight loss associated with GLP-1 medications
- significant calorie restriction
- inadequate protein intake
- major hormonal changes
- certain medications
These are all recognized triggers or potential contributors to excessive shedding. Dermatologists have also specifically reported hair thinning in people taking GLP-1 medications and believe sudden or substantial weight loss may be an important contributor. Eating substantially less can create additional nutritional stress. A direct drug effect on the follicle has not been ruled out, so rapid weight loss should not be presented as the only possible mechanism.
When the triggering stress resolves, follicles generally retain the ability to return to normal growth. The frustrating part is timing: because affected hairs have already shifted into the resting phase, reducing the stress today does not necessarily stop shedding tomorrow. Excessive shedding commonly becomes noticeable months after the triggering event.

Sensoril® Ashwagandha
Ashwagandha has randomized, placebo-controlled human evidence for helping regulate both the psychological and physiological response to chronic stress.
Sensoril® is a standardized ashwagandha root-and-leaf extract studied at relatively low doses. In a randomized, double-blind, placebo-controlled study of chronically stressed adults, participants taking 125 mg once daily for 60 days experienced a 62.2% reduction in modified Hamilton Anxiety scores, reflecting a substantial improvement in measured stress and anxiety symptoms.
At the same 125 mg daily dose, serum cortisol decreased approximately 14.5%. The study also found significant improvements versus placebo in several other stress-related measures.
Those results are relevant to stress-related shedding because chronic physiological stress can contribute to disruption of the normal hair-growth cycle. The Sensoril® study did not measure hair growth or telogen effluvium, so the 62.2% result should not be interpreted as a 62.2% improvement in hair. Its relevance is further upstream: helping address a physiological stress response that may contribute to excessive shedding.
